
Standards Procedure (Skill)
Clinical Indications: Level: EMT-P only
Ř Life threatening illness or injury in a child < 6 years of age (72 months) after effective ventilation is established
Procedure:
a. Expose the lower leg.
b. Identify the tibial tubercle (bony prominence below the knee cap) on the proximal tibia. The insertion location will be 1-2 cm (2 finger widths) below this and medially.
c. Prep the site as per a peripheral IV line.
d. Holding the intraosseous needle perpendicular to the skin, twist the needle handle with a grinding motion applying a controlled downward force until a “pop” or “give” is felt indicating loss of resistance. Do not advance the needle any further.
e. Remove the trochar and dispose of properly.
f. Attach the IV line to the IO catheter.
g. Stabilize and secure the needle with a hemostat, 4X4 pads and tape.
h. Document the procedure, time, and result on the ACR.
Certification Requirements:
Successfully complete an annual skill evaluation including the indications, contraindications, technique, and possible complications of the procedure.
