Standards Procedure (Skill)
Clinical Indications: Level: EMT-P only
Ø Arterial blood gas (ABG) analysis
Ø Other needs for arterial blood as indicated by medical control
Procedure:
a. Assemble the kit, ice, alcohol wipes, and gloves.
b. Determine if there is any history of trauma or any other difficulties with either hand. If a problem does exist, do not use that extremity for the blood draw.
c. Perform an Allen’s test on the selected extremity.
d. Palpate the radial pulse just proximal to the wrist.
e. Clean the skin with an alcohol wipe and allow the skin to dry.
f. Insert the ABG syringe at a 45° to 60° angle over the area of the pulse.
g. Slowly advance the syringe, watching for a return of arterial blood. You do not need to aspirate but instead allow the fill from the arterial pressure.
h. Once the sample has been acquired, remove and discard the needle in an approved fashion.
i. Place the small airtight cap over the needle port on the syringe. Remove air from the sample by inverting the syringe and pressing the plunger on the syringe until a small amount of the sample enters the airtight cap.
j. Place the sample on the ice as soon as possible.
k. Hold pressure over the blood draw sight for at least 5 minutes before checking the site to verify bleeding control.
l. Record procedure, time, and any complications in the ACR.
Certification Requirements:
Successfully complete an annual skill evaluation including the indications, contraindications, technique, and possible complications of the procedure.
