VERAPAMIL (CALAN)
Class: Calcium channel blocker
Mechanism Of Action: Vascular dilation and slows conduction through the AV node. Inhibits calcium ion influx across cell membrane during cardiac depolarization. Decreases rapid ventricular response seen with atrial tachydysrhythmias such as atrial fibrillation and atrial flutter. Dereases myocardial demand. Negative inotropic effects. Coronary and peripheral vasodilation.
Indications: PSVT refractory to adenosine. Atrial fibrillation and atrial flutter.
Contraindications: Severe hypotension. Cardiogenic shock. V-tach. Wolf-Parkinson-White syndrome. Severe CHF.
Precautions: Monitor BP constantly. Hypotension. Renal disease.
Side Effects: Hypotension. Asystole. Heart Block. Nausea/Vomiting. Bradycardia. CHF. Headache.
Dosage:
2-2.5 mg IV push over 2 minutes.
After 15-30 minutes, 5-10 mg IV
Max Dose: 30 mg in 30 minutes
Antedote: Calcium Chloride: 0.5 – 1.0 gm over 5 minutes slow IV push.
