HEPARIN

 

Class: Anticoagulant, antithrombotic

 

Mechanism Of Action: Prevents conversion of fibrinogen to fibrin prothrombin to thrombinby enhancing inhibiting effects of antithrombin III

 

Indications: Deep vein thrombus. Pulmonary embolus. MI. Open Heart Surgery.

 

Contraindication: Hemophilia. Leukemia. Peptic ulcer disease. Severe hepatic or reanl disease. Severe hypertension. Hypersensitivity.

 

Precautions: Alcoholism. Elderly. Pregnancy

 

Side Effects: Hepatitis. Thrombocytopenia. Fever. Diarrhea. Anaphylaxis. Hematuria.

 

Dosage:

DVT/MI
IV Bolus 5000-7500 units, then IV infusion 1000 units/hour titrated to PTT level

 

PE:
IV Bolus 7500-10,000 units, then IV infusion 1000 units/hour titrated to PTT level
