BRETYLIUM
Class: Antiarrhythmic
Mechanism of Action: Causes release of norepinephrine from adrenergic nerve endings, which in turn causes slight increase in heart rate, blood pressure, cardiac output. Then norepinephrine release is inhibited which results in adrenergic blockade then hypotension may develop. It elevates ventricular threshold. Sometimes converts ventricular fibrillation or ventricular tachycardia to supraventricular rhythms.
Indications: Ventricular fibrillation/ventricular tachycardia refractory to Lidocain.
Contraindications: None in life threatening emergencies.
Precautions: Keep patient supine due to possible hypotension.
Side Effects: Dizziness, Syncope, Seizures, Hypertension, Hypotension, Vomiting, Angina
Dosage: 5mg/kg IV bolus - After 5 minutes 10mg/kg – Max 30mg/kg
